AXA Health Insurance: The Complete Independent UK Review
AXA Health is one of the UK's Big Four private medical insurers, providing cover for over 2.8 million people. This independent guide examines every aspect of AXA Health cover — from personal and business policies to hospital networks, underwriting options and how they compare against other leading UK insurers.
Who Are AXA Health?
Founded in 1938 as the London Association for Hospital Services (later known as Private Patients Plan or PPP), the company is now a vital part of the French multinational AXA Group. Following a major rebrand in October 2020, they operate simply as AXA Health.
Today, they stand firmly as one of the UK's "Big Four" private medical insurance (PMI) providers alongside Bupa, Aviva, and Vitality, supporting more than 2.8 million members across the country.
They are widely recognised for quality, holding a Defaqto 5-star rating on both their Personal Health and Business Health plans, and maintaining a strong Trustpilot score of 4.1 out of 5 from over 21,000 independent reviews.
It's worth understanding that AXA distributes different products through different channels. Their flagship Personal Health and Business Health plans — both Defaqto 5-star rated — are available through independent brokers and advisers. Their direct-to-consumer AXA Health Plan, sold through the AXA website, carries a 4-star Defaqto rating and operates on a different structure. This guide covers both, highlighting where the products differ so you can make an informed decision.
If you're unsure whether they're right for you, explore our insurance guides or our independent advisors can help you compare your private health insurance options across the entire market.
Why Choose AXA Health?
AXA Health stands apart through four key strengths that consistently attract individuals and businesses.
Modular Flexibility
AXA offers flexibility across their product range. The direct-to-consumer AXA Health Plan uses a fully modular build-your-own approach, while the broker-distributed Personal Health plan provides a traditional core of inpatient and day-patient cover with optional add-on modules for outpatient, mental health, therapies, dental and optical.
Doctor@Hand
24/7 access to virtual GP appointments through Doctor Care Anywhere. Prompt video or phone consultations with prescriptions available — included on all personal and business plans.
Comprehensive Cancer Cover
Available as an optional module, AXA's cancer cover has no financial cap on eligible treatment including surgery, chemotherapy, radiotherapy, targeted therapy and immunotherapy. It extends to licensed cancer drugs not available through the NHS. An NHS Cancer Support option is also available for those who prefer primary treatment through the health service.
No Claims Discount
Industry-leading 17-level NCD scale offering up to 80% discount. Claims reduce by only 3 levels — and on the direct AXA Health Plan, claims totalling under £250 in a policy year don't affect your discount at all.
Types of AXA Health Cover
Personal Health (Broker-Distributed): AXA's flagship plan for individuals and families, available through independent advisers. It provides core inpatient and day-patient cover as standard, with optional modules for outpatient care, cancer cover, mental health, therapies, dental and optical. This plan holds a Defaqto 5-star rating.
AXA Health Plan (Direct Only): Sold exclusively through the AXA website, this plan uses a more modular building-blocks approach where cover is assembled from individual components. It carries a Defaqto 4-star rating and is only available with moratorium underwriting — Full Medical Underwriting is not offered on this product. Uniquely, this plan allows different family members on the same policy to have entirely different cover options.
Health for You: An older or specific corporate/scheme product for individuals and families, available through brokers, with a more restricted hospital list. This plan carries a 4-star Defaqto rating.
Family Cover: Family policies are available across most AXA products, with individual No Claims Discount scales for each family member. AXA notably allows multi-generational family cover, supporting up to four generations of a family on a single policy.
Standalone Cancer Cover: Uniquely in the UK market, cancer cover can be purchased as a standalone product through AXA without needing to buy a core inpatient health insurance plan first.
Business Health Insurance: For companies, the Business Health plan caters to SMEs with 1 to 249 employees, while Business Protect offers a more budget-conscious alternative with a restricted hospital network. Both are 5-star Defaqto rated and available through brokers.
Corporate & Executive: AXA Advance, designed for organisations with 250 or more employees, is typically only available through brokers and specialist consultants. Medical History Disregarded underwriting is available from 15 employees. These policies often align with the needs of company directors, contractors, and self-employed professionals.
Hospital Networks and Consultant Choice
AXA Health partners with over 250 private hospitals across the UK. However, the specific facilities you can access depends heavily on which of the three distinct hospital lists applies to your policy, particularly regarding access to Spire and Ramsay networks:
The Directory of Hospitals (Main National List)
Used on: Personal Health (Broker) and the AXA Health Plan (Direct Standard)
This tier offers full, unrestricted national access to Spire Healthcare and Ramsay Health Care facilities across the UK, including outer-London clinics. To unlock elite, high-cost central London facilities, you must add the Greater London Upgrade.
The Health for You List
Used on: Health for You — available via brokers for individuals (not corporate)
A nationwide hospital list that includes Nuffield Health and Circle Health Group facilities as its core network. Also features a select handful of Spire hospitals (e.g., Norwich, Cardiff). Provides broad national coverage for individual policyholders through broker-arranged plans.
The Specified Hospital List
Used on: The Direct Foundation policy
Stripped down strictly to regional hubs. Excludes smaller day-patient clinics and focuses only on major regional surgical hospitals to cover core surgical needs while reducing premium cost. Elite central London private hospitals are entirely excluded.
The Inspire Plan (Spire Network)
Used on: Specially designed for the Spire Healthcare Group
Provides access to all Spire Healthcare hospitals across the UK, along with a select handful of Ramsay facilities. Purpose-built for the Spire network.
Consultant Choice Options
In addition to your hospital list, you also choose how you access consultants:
Guided Option
AXA selects a suitable specialist for you from up to 3 options, simplifying the pathway and reducing overall costs.
Consultant Choice
Maximum freedom. You pick your own consultant from the directory, giving you total control at a higher premium tier.
Open Option
The most comprehensive tier — access any private hospital in the UK, including those outside AXA's standard directory, at a premium cost.
Underwriting Options
When applying for cover, the underwriting method determines how your medical history is treated. It's important to note that underwriting options differ between AXA products:
Moratorium Underwriting: Available across AXA's product range, though the exact look-back period and qualifying terms can vary between their different products. The direct-only AXA Health Plan features a shorter THREE-YEAR look-back period than the industry-standard five years, while the broker-distributed Personal Health plan may apply standard terms. Under moratorium, pre-existing conditions are not covered initially but may become eligible after a continuous trouble-free period.
Full Medical Underwriting (FMU): Available on the Personal Health plan through brokers. Your conditions are assessed individually through a medical questionnaire upfront, providing clarity on exactly what is and isn't covered from day one. Notably, the direct-only AXA Health Plan does not offer this option.
Medical History Disregarded (MHD): For corporate schemes, AXA offers MHD underwriting from 15 or more employees, allowing eligible pre-existing conditions to be covered without individual medical assessments.
Understanding which underwriting route is available — and most suitable — for your circumstances is one of the strongest reasons to seek independent advice rather than purchasing directly.
Outpatient Cover Options
AXA drops the cash-pool model used by Vitality and Bupa, choosing instead to restrict outpatient care by the number of consultations.
AXA Health Plan (New Direct)
Offers an option for Full Outpatient Cover with unlimited consultations and unlimited routine diagnostics like bloods and X-rays. Advanced MRI, CT, and PET scans are always paid in full.
Personal Health (Broker)
Features an option for 3 specialist consultations per year. While specialist appointments are capped at three, diagnostic tests and advanced scans remain completely unlimited.
Direct Foundation
Operates on the same structure as other AXA plans, offering either a standard outpatient tier (3 specialist consultations plus unlimited diagnostics) or a full outpatient tier (unlimited everything, no annual limits).
The exact outpatient structure available to you depends on which AXA product you are considering. An independent adviser can model the different options and their impact on premium to help you decide. For a broader understanding of how private medical insurance works, our comprehensive guide covers all the key concepts.
Excess Options
AXA offers excess levels from £0, £100, £250, and £500 on their standard (mid-level) outpatient option. If you opt for the full outpatient cover, you also gain access to higher excess levels of £1,000, £2,500, and £5,000. The excess applies once per person, per policy year — so regardless of how many separate claims you make within that year, you only pay the excess once.
Selecting a higher excess will reduce your monthly or annual premium, but it does mean a larger out-of-pocket contribution at the point of claim. Our advisers can model these figures alongside different outpatient and hospital list combinations to identify the most cost-effective structure for your circumstances.
Understanding the No Claims Discount
AXA uses a 17-level no claims discount scale — one of the most generous in the UK private health insurance market. This foundational structure is shared across all three of AXA's personal product lines: Personal Health, Health for You, and the direct-to-consumer AXA Health Plan.
The mechanics are straightforward: each claim-free year earns you a higher discount, moving up one level at a time to a maximum of 80% off your premium at Level 17. If you do make an eligible claim, your discount drops back by three levels at renewal — but crucially, it never resets to zero entirely. And regardless of how much a claim costs or how many claims are made within a single policy year, your discount only ever drops by a maximum of three levels.
On a family policy, each person has their own independent NCD level. One family member claiming does not affect anyone else's discount on the plan.
The £250 Safe Zone — AXA Health Plan Only A critical distinction exists between AXA's product ranges. The direct-to-consumer AXA Health Plan includes a £250 small-claims buffer: if the total value of all claims made during a policy year is under £250, your NCD level is completely unaffected at renewal. This means a minor initial consultation or quick diagnostic test won't trigger the three-level penalty.
The broker-distributed Personal Health and Health for You plans do not feature this generic small-claims threshold. On these policies, any paid claim — regardless of value — triggers the standard three-level drop. The only exceptions are specific exempt benefits such as using the NHS Cash Benefit, or claims made under standalone dental, optical, or therapies modules.
NCD protection is also available as an optional add-on across AXA's range for those who want extra peace of mind. Our expert advisers can explain exactly how the NCD interacts with different excess and cover options to help you maximise your long-term value.

Mental Health Support
All AXA Health plans include a 24/7 health support line — staffed by trained nurses and counsellors — along with the Mind Health Self-check digital assessment tool at no additional cost. These provide initial guidance and signposting rather than clinical treatment.
The dedicated Mental Health module, available as an add-on, unlocks direct access to counsellors, psychologists and psychiatrists without requiring a GP referral. It covers talking therapies including cognitive behavioural therapy (CBT) and structured counselling sessions.
It's worth noting that AXA's mental health provision is generally considered less comprehensive than Bupa's, which offers broader direct-access clinical pathways and higher session limits. AXA also does not typically offer the mental health module to individuals who have been referred to a psychiatrist within the previous five years.
For self-employed professionals and business owners, mental health support can be critical to maintaining productivity. Our guide on NHS versus private healthcare explores the wider picture.
Optional Extras & Member Benefits
- Dental and Optical Cashback
Available as an optional add-on. The standard tier offers 80% cashback up to £400 for dental work, £200 for optical care, plus £25 towards an eye test. A Plus tier is also available with higher limits of up to £500 dental and £300 optical at 100% reimbursement.
- Wellness Rewards
Benefit from 40% off Nuffield Health gym memberships, Hussle multi-gym discounts, and 25% off health assessments at Circle and Nuffield clinics.
- NCD Protection & Additional Benefits
NCD protection is available to preserve your discount. Cover also includes a parent accommodation benefit if a child is receiving inpatient treatment, and an NHS cash benefit of £50 per night if you choose to receive treatment through the NHS instead of using your private cover.
How AXA Health Claims Work
The claims process with AXA is designed to be highly accessible and digitally integrated:
- Get a Referral: Obtain an open referral from your NHS GP or via the Doctor@Hand digital service.
- Pre-authorisation: Contact AXA via your online account, phone, or live chat before proceeding with treatment.
- Receive Approval: Once eligible, AXA will generate your unique authorisation number.
- Book Appointment: Schedule your session with a registered specialist. Note: If using the Guided Option, AXA will select the specialist for you.
- Attend Clinic: Bring your membership number, authorisation number, and referral letter to your appointment.
AXA Health Strengths

- No Claims Discount up to 80% across 17 levels — industry-leading retention of earned discount
- Doctor@Hand 24/7 virtual GP included on all plans
- Cancer cover module with no financial cap on eligible treatment
- Standalone cancer cover option — unique in the UK market
- Multi-generational family policies covering up to four generations
- Shorter three-year moratorium on the direct AXA Health Plan
- Competitive pricing, particularly for SME group schemes
- Strong digital experience with new all-in-one app
- MHD available from 15 employees on corporate schemes
- Defaqto 5-star rated Personal Health and Business Health plans
AXA Health Considerations
- Strict cancer history exclusion — the AXA Health Plan excludes all previous cancers regardless of how long ago they occurred or were resolved, which is significantly stricter than some competitors
- Mental health cover restricted — not available to anyone referred to a psychiatrist within the five years before joining, and only covers new conditions developing after the policy starts
- Mental health cover less comprehensive than Bupa's direct-access model
- Smaller hospital network compared to Bupa
- Cancer cover is an optional add-on module, not included as standard
- Different products available through different channels can create confusion — independent advice recommended
- Customer service rated lower than WPA in independent surveys
- Guided Option removes choice of consultant
- Direct-only AXA Health Plan does not offer Full Medical Underwriting
Who Is AXA Health Best Suited For?
AXA Health's unique architecture makes it particularly well-suited for several demographics:
- Individuals who want granular control over their policy modules.
- Self-employed professionals and contractors seeking ultimate flexibility.
- SME business owners looking for highly competitive group rates.
- People with a clean health history who can fully leverage the lucrative No Claims Discount scale.
- Company directors structuring tax-efficient corporate cover.
- Families wanting family cover that protects individual NCDs for each member.
However, if mental health support is your absolute primary concern, Bupa may represent a stronger fit. If unparalleled customer service is your priority, WPA frequently secures the top spot. If deep wellness rewards and active lifestyle incentives appeal to you, Vitality leads the market.
Because AXA distributes different products through different channels — each with varying features, underwriting options and hospital lists — navigating the range without professional guidance can be difficult. This is one of the strongest cases for using an independent adviser who understands the full product architecture, rather than relying solely on the consumer website. If you're considering switching from an existing policy, expert advice becomes even more critical.
What's Not Covered by AXA Health?
As with all private medical insurance, AXA Health policies are designed to cover acute, short-term conditions rather than ongoing maintenance. Standard exclusions across their plans include:
- Chronic (long-term) conditions
- Dental treatment
- Eye conditions
- Learning and developmental disorders
- Cosmetic procedures
- Kidney dialysis
- Fertility treatment
- Treatment for alcohol, drug, or substance abuse
- Self-inflicted injury
- Weight loss treatment
There are also some important AXA-specific notes to be aware of:
- Previous Cancers: AXA excludes all previous cancers regardless of when they occurred — this is a notable policy rule.
- Mental Health: Mental health cover is not available to anyone who has been referred to a psychiatrist in the past five years.
- Pre-existing Conditions: Pre-existing conditions may be excluded depending on the underwriting method.
Customer Reviews
AXA Health maintains a strong reputation among its members, holding a Trustpilot score of 4.1 out of 5 from over 20,000 reviews, with over 5,000 reviews in the last 12 months.
Positive themes: Many members highlight efficient service, quick diagnosis, and knowledgeable and friendly staff.
Areas for criticism: Some criticism notes occasional claims issues, long call wait times, and the online system not always resolving queries.
Independent customer reviews are one factor to consider alongside personal suitability, exclusions, and overall policy benefits.
